Patient understands that Physician has two locations. Patient may be asked to see the physician at
either 1545 East Southlake Blvd., Ste 110, Southlake, TX 76092 or 1110 Cottonwood Lane, Irving, TX 75038
in the case of an emergency visit. All imaging services are to be done at the Irving location.
The patient understands that the physician will not bill insurance, but will provide the patient with
the proper paperwork for self-filling if requested.
